Supports begin
Once everyone's comfortable, we set up a service agreement and match the participant with the right support team.
Who can refer
Referrals welcome from anyone
You don't need to be a professional to make a referral. We accept referrals from:
Family members, carers and friends supporting someone with a disability.
Support coordinators and Local Area Coordinators (LACs) connecting participants with providers.
Doctors, allied health and other health professionals involved in someone's care.
Hospital discharge and care teams arranging supports for a safe transition home.
Participants themselves — you're always welcome to reach out on your own behalf.
